Give us a call at 800.367.3937 (9am – 6pm ET Monday - Friday) or email customerservice@foreyes.com. Do not worry if you cannot access your Member ID - we can verify your benefits without it.

OR to submit your own claim directly with your insurance company:

Complete the claim form above and submit it with your itemized receipt to this address:

Step 2: Include itemized receipt

You will receive an itemized receipt with your order confirmation email and with your package. You can also request a receipt from our customer service team by giving us a call at 800.367.3937 (9am – 6pm ET Monday - Friday) or sending an email to customerservice@foreyes.com.

Step 3: Submit claim form and receipt to your insurance company

Please note: receipts for single-vision glasses with polycarbonate lenses do not break down the cost of frames and lenses separately because they are a packaged item. If your insurance provider has questions, please do not hesitate to reach out to us.

Step 4: Get money back

After submitting your claim, you will receive your reimbursement directly from your insurance company in about 2-3 weeks, depending on the plan. Good news: Most insurance plans will cover most or all your For Eyes purchase!
